Scanned documents are the most common source of upside-down PDFs. The scanner creates an image-based PDF where each page is essentially a photograph of the paper. The orientation depends entirely on how the paper was fed.

For a single-sided scan where everything is upside down: use "Rotate All 180 degrees."

For a duplex scan where every other page is flipped: click each inverted page thumbnail twice (180 degrees per page). This is manual but accurate. For PDFs under 30 pages, it takes less than a minute.

After fixing the orientation, you might also want to run the document through PDF OCR to make the scanned text searchable and copyable. Rotation should happen first — OCR accuracy improves when the text is right-side up.

A common frustration: you rotate a PDF in a viewer, close the file, reopen it, and it is upside down again. This happens because the viewer rotated the display but did not change the file.
